) All athletes wanting to participate in Junior High football this fall must have all paperwork turned in, including the following:
1. MPA Athletic Packet (available in the front office)
2. AIA Physical (required every calendar year)
a. Physical form (page 1)
b. Physical form (page 2) 3. Fees (Fees for all junior high sports are $150, please make a check/money order out to Mesa Prep Athletics)
4. A copy of your insurance card on file with the athletic office
Practice Schedule:Practices before first game: Monday, Wednesday – 3:45-5:15pm; Saturday – 9:15-10:45am Practices after first game: Monday, Wednesday, Friday – 3:45-5:15pm *All practices will be held at the Mesa Prep field: 1303 S. Lindsay Road, Mesa, AZ 85204
